Healthy Ageing – A Nurse led early intervention program

General Practices are invited to participate in the new ‘Healthy Ageing’ Program – building capacity within general practice to deliver nurse-led care to patients to support healthy ageing and improve access to palliative care.

Telehealth in Aged Care

These telehealth modules are available free of charge to clinicians and RACH staff within the Healthy North Coast region.
You will need to create an account and log in to access these modules.
These modules are part of the National Telehealth Training program, developed by PHNs and are endorsed by Royal Australian College of General Practitioners, Australian College of Nursing and Australian College of Rural and Remote Medicine.
