Healthy Ageing Early Intervention Program
A Nurse Led Collaborative Program for General Practices
General Practices are invited to participate in the new ‘Healthy Ageing’ Program – building capacity within general practice to deliver nurse-led care to patients to support healthy ageing and improve access to palliative care.
This collaborative program focuses on early intervention in chronic disease, multimorbidity, living with dementia, and end of life care. It will incorporate new Health Pathways, lifestyle medicine, health literacy, and coaching. It will be led by your own skilled practice nurses.
Participating practices will receive comprehensive education, tools, and resources to support their involvement.
The program is being delivered by Primary Care Innovations and funded by Healthy North Coast through the Australian Government’s Primary Health Network program. Primary Care Innovations is a consortium consisting of MediCoach, Medical Business Services, Practice Coach Australia and Aria Health.
We’re committed to supporting your practice to enhance care to patients from midlife and beyond or those receiving palliative care. Five practices across the Mid North Coast and North Coast will be selected to be part of the initial collaborative network, working with us over a 12-18 month period.
You can expect to
- learn new strategies for utilising Health Pathways,
- deliver Early Intervention programs,
- implement new ways of nurse-led working,
- develop a robust Quality Improvement Action Plan,
- undertake targeted education and training
- create a template for ongoing success.
In addition, financial incentives of $12,000 per participating pilot practice are available to support the seamless integration of this program into your practice.
