RACH Building Nurse Capacity Project

We’re inviting residential aged care facilities to join a fully-funded pilot program focused on building nurse capacity and improving care for older residents through targeted, practical education and coaching.

This program is a collaboration between Practice Coach and ARIA, delivered with support from the Healthy North Coast PHN program. It offers short, accessible learning modules – designed with direct input from aged care staff – to address real challenges in areas like:

  • Recognising and responding to a deteriorating resident (DRTT)

  • Using My Health Record effectively

  • Advanced care planning and palliative care

  • Telehealth and anticipatory care practices

Alongside the education, you’ll receive hands-on support and coaching to implement what you’ve learned, participate in quarterly Communities of Practice, and connect with pilot general practices that are developing nurse-led models of care.
